Below is a list of records that are publicly available pursuant to Executive Law §94(19).

1. Financial Disclosure Statements for certain executive and legislative branch employees and officials, not including categories of value. Effective until January 1, 2013.

2. Financial Disclosure Statements for certain executive and legislative branch employees and officials. Beginning January 1, 2013.

3. Notices of Financial Disclosure Statement filing delinquencies pursuant to Executive Law §94(12).

4. Notices of civil assessments, or settlement agreements which include a fine, penalty, or other remedy.

5. Documents required to be disclosed pursuant to Legislative Law Article 1-a (the Lobbying Act).

6. Substantial Basis Investigation Reports issued by the Commission pursuant to Executive Law §94(14-a) and (14-b).

Please grant access to the following records in accordance with Executive Law §94(19)(a) and the Commission regulations for access to publicly-available records.

If it is necessary to photocopy the requested records, I understand that the Commission waives copying fees unless the request is for more than 40 pages, in which case the Commission shall charge $0.25 per copy or the cost of electronic reproduction.
